Dr. Jefferson U. Davis has announced his retirement and closure of his practice effective June 30, 2012.

 Our medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your written permission.

 Before June 30th, 2012: Patients may obtain copies of their records or request transfers at the office.

 After June 30th, 2012: Patients can submit written request to P.O. Box 72228, Albany, GA 31728.
